Equinococosis ósea extraarticular en fémur distal. Reporte de caso

RESUMEN
Introducción: la equinococosis es una
enfermedad parasitaria infrecuente que afecta principalmente
hígado y pulmones; la afectación ósea es excepcional.
La escasa literatura sobre compromiso óseo dificulta
la sospecha diagnóstica. Las lesiones típicamente quísticas
requieren resección quirúrgica urgente para confirmación
diagnóstica e inicio de tratamiento antiparasitario con albendazol,
evitando así el riesgo de diseminación.
Caso
clínico: paciente femenino procedente de Medio Oriente,
quien presenta dolor crónico en rodilla izquierda que se
exacerba luego de traumatismo. Los estudios revelan quiste
equinococal óseo. Se realizó resección intralesional con
cementoplastía impregnada con albendazol, complementada
con albendazol oral adyuvante. La paciente presenta
mejoría sintomática sin recurrencia.
Conclusiones: dada la
rareza de la equinococosis ósea es fundamental reconocer
sus manifestaciones clínicas y radiológicas para establecer
el diagnóstico temprano. El tratamiento quirúrgico mediante
resección del quiste constituye la primera línea terapéutica,
complementado con antiparasitarios orales para prevenir recurrencias
locales. En pacientes no candidatos quirúrgicos,
el tratamiento médico reduce el tamaño quístico y mejora la
calidad de vida. El seguimiento prolongado es esencial para
detectar y tratar precozmente posibles recurrencias.
