Exposición a biomasa y su asociación con la enfermedad pulmonar obstructiva crónica

RESUMEN

Introducción: La exposición a humo de biomasa se ha considerado un factor de riesgo para enfermedad pulmonar obstructiva crónica, este estudio evalúa dicha asociación dependiendo de los años de exposición.
Objetivo: Evaluar la exposición a contaminantes de biomasa en cuanto a duración de esta y su impacto en la función respiratoria de los pacientes expuestos.
Métodos: Estudio de corte transversal con componente analítico en pacientes que asistían a consulta externa por cualquier causa a una clínica de tercer nivel de atención en Colombia entre los meses de agosto de 2017 a junio de 2018.
Resultados: Se incluyeron en el análisis final 1997 sujetos, 23 % con diagnóstico previo de enfermedad pulmonar obstructiva crónica; edad promedio de 65 años, 59 % de los sujetos con antecedentes de exposición al humo de biomasa, con promedio de exposición de 21 años. El 47 % tuvo antecedente de tabaquismo, con un índice de paquetes año promedio de 15. Del total de sujetos estudiados, 83 % presentaba síntomas respiratorios.
Conclusiones: Existe relación estadísticamente significativa entre la exposición al humo de biomasa y la enfermedad pulmonar obstructiva crónica a partir de los 20 años de exposición, ajustado por edad, sexo y antecedente de tabaquismo. En los pacientes con exposición al humo de leña y confirmación de la enfermedad pulmonar obstructiva crónica, predomina un grado moderado de obstrucción de acuerdo con los hallazgos espirométricos con base a los parámetros de la guía GOLD. No se encontró relación entre la presencia de enfermedad pulmonar obstructiva crónica y las horas de exposición diaria.
