La incidencia de aloinmunización anti-RD en personas
RhD-negativo que reciben una transfusión
de componentes celulares RhD-positivo es variable,
con un rango general estimado entre el 12 y 80%,
dependiendo del estado clínico e inmunológico
del paciente. Tan solo una pequeña cantidad de
glóbulos rojos RhD+ (alrededor de 0.1 a 0.5 mL)
puede iniciar la respuesta inmunológica, de modo
que la administración de una unidad de glóbulos
rojos RhD+ podría elevar la probabilidad de formar
anticuerpos anti-RhD hasta en un 80% en pacientes
inmunocompetentes. Desde el punto de vista
clínico, la presencia de estos anticuerpos puede
causar futuras reacciones hemolíticas de tipo tardío
o enfermedad hemolítica feto-neonatal (EHFN).
