El conocimiento actual sobre el cáncer de recto es el resultado del trabajo acumulado de numerosos investigadores y de décadas de investigación clínica. Sin embargo, los últimos 25 años (y de manera particularmente marcada la última década) han sido profundamente disruptivos, transformando no sólo nuestra comprensión de la enfermedad, sino también su abordaje terapéutico.
Hasta finales del siglo XX, la cirugía se consolidó como el pilar del tratamiento curativo del cáncer de recto. La contribución histórica del Dr. Bill Heald permitió la estandarización de la técnica quirúrgica mediante la correcta identificación del plano sagrado y la implementación sistemática de la escisión total del mesorrecto (Total Mesorectal Excision, TME), con un impacto decisivo en la reducción de la recaída local y en la mejora de los resultados oncológicos.
