Fracturas subtrocantéricas de fémur tratadas con enclavado endomedular bloqueado

RESUMEN

Objetivo: Determinar la eficacia del tratamiento de fracturas subtrocantéricas de fémur con enclavado endomedular bloqueado. Materiales y métodos: Estudio descriptivo, observacional, retrospectivo: Serie clínica, donde se estudiaron los resultados de diecinueve pacientes con diagnóstico de fractura subtrocantérica durante los años 2000-2004 en diversos hospitales. Se designó la fractura de acuerdo a los criterios AO y a la clasificación de Russell-Taylor. La valoración final se describe de acuerdo a escala de evaluación de la cadera traumática de Sanders y cols. Resultados: Trece pacientes masculinos, seis femeninos. El promedio de edad fue de 36 años. El mecanismo fue por accidentes de tránsito y heridas por arma de fuego. El promedio de seguimiento fue de 18 meses. El tiempo de consolidación de las fracturas fue de 3 meses. Dos pacientes consolidaron en varo (‹ 5º) y presentan acortamiento menor de 2 cm. De acuerdo al puntaje de la escala de Sanders y cols. se reportaron 10 resultados excelentes, 8 resultados buenos y uno regular. Conclusiones: Recomendamos la colocación del clavo endomedular bloqueado monoblock en fracturas subtrocantéricas.
