Evaluación por tomografía computarizada de diastasis de la sindesmosis en fracturas de tobillo tipo B y C de AO/OTA

RESUMEN
Antecedentes: Hay variantes anatómicas en la sindesmosis tibioperonea que provocan que los criterios radiográficos de lesión de la misma no sean constantes.
Objetivo: Determinar diastasis sindesmal mediante tomografía computarizada (TAC) en fracturas de tobillo tipo B y C (AO/OTA) y relacionar las variantes anatómicas en la incisura tibial con la separación del peroné.
Material y métodos: Escrutinio comparativo, transversal y prolectivo que incluyó a todos los pacientes con fractura de tobillo tipo B y C (AO/TA) que completaran su evaluación radiográfica y por TAC. Los parámetros radiográficos fueron el espacio claro tibioperoneo, el solapamiento tibioperoneo y el espacio claro interno; por TAC se midió la longitud, profundidad y forma de la incisura tibial. Resultados: Fueron 17 pacientes: 8 mujeres (47%) y 9 hombres (53%), con una edad promedio de 41 años. Catorce (82%) con fractura a nivel sindesmal y 3 suprasindesmales. Por TAC se encontraron 10 incisuras cóncavas, 4 irregulares y 3 planas. La profundidad media de la incisura fue de 2.82 ± 0.89 mm, anchura promedio de 22.18 ± 3.04. No hubo diastasis sindesmal radiográficamente en 6 casos (35.2%) y por TAC sólo 3 (17.7%) de éstos. El diagnóstico de diastasis sindesmal se hizo en 11 por radiografías simples (64.8%) y con TAC en 14 (82.3%) (p = 0.043).
Conclusión: La radiografía es cuestionable para definir la diastasis sindesmal, por ello la TAC debe ser considerada cuando el diagnóstico es dudoso.
