Tratamiento paliativo endoscópico de obstrucción duodenal maligna

RESUMEN

Introducción: El tratamiento quirúrgico es considerado el método paliativo estándar para restaurar la continuidad luminal en la obstrucción duodenal maligna. En pacientes que no son candidatos a cirugía paliativa, el tratamiento endoscópico con instalación de prótesis metálicas autoexpandibles ha proporcionado una opción alternativa. Presentación del caso: Hombre de 48 años de edad con obstrucción del tracto de salida gástrico secundaria a estenosis duodenal por recidiva de un adenocarcinoma de la unión esofagogástrica, con manejo paliativo endoscópico mediante instalación de prótesis metálicas autoexpandibles no cubiertas. Conclusiones: La colocación endoscópica de las prótesis metálicas autoexpandibles constituye una alternativa terapéutica en el manejo paliativo de la obstrucción duodenal maligna en pacientes de alto riesgo. Evita un procedimiento quirúrgico mayor y reduce de manera significativa la morbilidad y la estancia hospitalaria del paciente.
