RESUMEN

Se realizó el reemplazo total de tres articulaciones temporomandibulares con el sistema de W. Lorenz en dos pacientes con anquilosis: la primera con anquilosis ósea, en la que había ausencia total de los movimientos mandibulares, condición que presentaba desde el año y medio de edad; la segunda con anquilosis fibrosa, por lo que presentaba limitación de la apertura oral de 12 mm, y condición posterior a fractura subcondílea no tratada, con tres años de evolución. La primera paciente fue sometida a reemplazo total de las dos articulaciones temporomandibulares mientras que en la segunda fue realizado solo el reemplazo de una articulación temporomandibular. El seguimiento, hasta el momento de este informe, es de ocho meses, con una apertura oral estable de 29 mm en el primer caso y de 34 mm en el segundo, sin desviación de la mandíbula durante la apertura ni dolor posoperatorio en ninguna de las dos pacientes. En ambos casos no hubo afectación del nervio facial de manera permanente. El reemplazo total de la articulación temporomandibular demostró ser efectivo y con resultados estables en pacientes con anquilosis de esa región.
