Fístula biliopleural en un paciente postrasplantado hepático. Una presentación inusual de complicación biliar

RESUMEN

Una fístula biliopleural es una complicación poco común posterior al trasplante hepático. Se presenta el caso de un paciente de 44 años con cirrosis por el uso de metotrexato e hígado graso no asociado a alcohol, con trasplante de donador por muerte encefálica, sin complicaciones. Reingresa por dolor abdominal, náusea y diarrea un mes después de la cirugía. Se encontró por radiografía y ultrasonido un derrame pleural de predominio derecho, el cual se drenó; sus características consistían con líquido biliar. Se realizó una colangiopancreatografía retrógrada endoscópica con esfinterotomía y colocación de endoprótesis plástica. A los dos meses de la colocación del stent reingresa por ictericia. Se identifica una estenosis y se realiza una dilatación con balón y recambio de prótesis. Dos meses después se retira la prótesis sin datos de fuga ni estenosis. La experiencia y la revisión de la literatura indican que es un padecimiento poco común que puede ser tratado sin necesidad de intervención quirúrgica, limitándose a radiología intervencionista.
