Biliopleural fistula in post-liver transplant patient. An unusual presentation of biliary complication

ABSTRACT

Biliopleural A fistula is an uncommon complication after liver transplantation. We report the case of a male patient aged 44 with cirrhosis due to methotrexate use and fatty liver not associated with alcohol and an uncomplicated liver donor with brain death. Readmitted for abdominal pain, nausea and diarrhea one month after surgery. Pleural effusion was found predominantly on the right by radiography and ultrasound which was drained and consisted with bile fluid characteristics. We performed endoscopic retrograde cholangiopancreatography with sphincterotomy and plastic stent placement, two months after stent placement readmitted for jaundice, stenosis is identified and performed balloon dilatation and prosthesis replacement. Two months later the prosthesis is removed without signs of leakage or stenosis. The experience and review of the literature, indicate that it is a rare condition and can be treated without surgery, with only interventional radiology.
