Enfermedad mamaria de Paget: actualización del tema

RESUMEN

La enfermedad mamaria de Paget (EP) fue descrita en 1874 por sir James Paget y es una manifestación superficial de enfermedad maligna de mama. Suele presentarse en 1-3% de todos los cánceres de mama y afecta principalmente a mujeres entre los 50-60 años. La forma clínica clásica se manifiesta con una placa infiltrada unilateral con eritema, escama, y en etapas avanzadas, con secreción o retracción del pezón. Es necesario realizar una biopsia para el diagnóstico preciso y descartar los diferenciales. Histológicamente se caracteriza por células de Paget, que son ovaladas, de citoplasma abundante y claro y que suelen estar suprabasales en la epidermis. La forma más frecuente es la que se acompaña de carcinoma intraductal. El tratamiento más aceptado es mastectomía radical más radioterapia con vigilancia estrecha. Presentan mejor pronóstico si no hay masa palpable y si el diagnóstico es temprano.
