Comparison of two surgical techniques for managing facial aging and used at ''Hermanos Ameijeiras'' clinical-surgical hospital

ABSTRACT

Introduction: Aging is a natural, unique and particular process for each individual. Although it is governed mainly by genetic load, there are multiple treatments that delay it. The outcomes are presented of the surgical managment of cervicofacial flaccidity with two surgical techniques.
Objective: To compare the surgical outcomes of facial rejuvenation by means of either cervicofacial rhytidectomy with plication or superficial musculoaponeurotic system dissection.
Method: A randomized, longitudinal, prospective and comparative study was carried out, for which the sample was divided into two groups. The universe was made up of all the patients with clinical signs of facial aging who attended the outpatient consultation of plastic surgery at Hermanos Ameijeiras Clinical-Surgical Hospital. Patients with prolonged use of nonsteroidal antiinflammatory drugs (NSAIDs), previous facial surgeries, congenital or acquired facial deformities, active dermatological diseases, decompensated systemic diseases, psychiatric diseases, Ehler-Danlos syndrome, keloids, without preoperative photographic control, smokers and pregnant women were excluded. The sample consisted of sixteen patients undergoing cervicofacial rhytidectomy. The group I (study) was made up of nine patients operated on by plication. The group II (control) was made up of seven patients operated on by dissection.
Results: There was a predominance of patients aged more than 60 years old, of the female sex, with mixed skin and stage III of facial aging in the preoperative period, as well as stage I of facial aging in the postoperative period in both groups. To assess the aesthetic outcomes, the nasolabial fold, mandibular lip, as well as the curvature of the cheek and neck were examined. In the patients from group I, the outcomes were good (77.7%) or fair (22.3%), while in the patients from group II, the outcomes were good (85.7%) or fair (14.3%). No complications or poor outcomes were identified.
Conclusions: In most patients, it was possible to decrease at least one stage in the degree of aging, with facial symmetry, little visible scar and without complication. No statistically significant differences were found.
