Risk factors associated with recurrence of discrete subaortic stenosis

ABSTRACT
Objective: The aim of this study was to analyze the risk factors associated with recurrence of discreet subaortic stenosis.
Methods: From July 1993 to July 1998 all patients with diagnosis of discrete subaortic stenosis were included. U Mann-Whitney, Ji Square and ANOVA test were applied, 5% of significance was used.
Results: 31 pediatric patients were divided into two groups. Group A (n = 13) had gradients less than 20 mmHg. Group B (n = 18) had gradients equal or more than 20 mmHg. There were no clinical differences between both groups. Eight patients in Group A and 13 in Group B underwent surgical membranectomy. A significant gradient was found in 9 patients. There was no early mortality. Recurrence was associated with a preoperative higher gradient (more than 20 mmHg), aortic regurgitation, younger age at surgery (p ‹ 0.05). Postoperative complications occurred in 7%.
Conclusions: Although surgical treatment provides very good results, recurrence is significant: 42%. Risk factor for recurrence was a higher preoperative gradient. Surgical membranectomy is recommended once the diagnosis is established. A careful postoperative follow up is indicated.
