Non-surgical treatment of periimplantitis: a narrative clinical review.

ABSTRACT

Currently, we have a global epidemiological problem for the presence of peri-implantitis; which can lead to aesthetic and functional problems. To treat this pathology we must begin with a hygienic phase that includes controlling the associated risk factors and starting with non-surgical therapy. However, the information provided so far is insufficient to give clear recommendations on what type of non-surgical therapeutic approach would be the most effective. The objective of this narrative clinical review is to update and describe each of the non-surgical therapeutic approaches to treat peri-implantitis; evaluating its clinical effectiveness. A bibliographic search of the literature published in the last 10 years was carried out, during the period of April and May 2020, in the Medline-PubMed and EBSCOhost databases; including only clinical trials with a minimum six-month follow-up in English. The literature reviewed focuses on testing different therapeutic approaches such as aero polishers, chlorhexidine, laser, photodynamic therapy, antibiotic therapy and probiotics; alone or in combination with manual mechanical therapy and/or ultrasonic; comparing its efficacy at a clinical and microbiological level, finding beneficial results in terms of reducing the clinical parameters of infection and inflammation; however, it was observed that no therapy used is completely superior to the other. Therefore, nonsurgical treatment with any type of well-performed combination of therapy, along with control of contributing risk factors; they generally provide a reduction in the clinical features of peri-implantitis.
