ABSTRACT

This is the case report of a patient with a perforated acute appendicitis that progressed for one week and resulted in peritonitis with secondary splenic tear. The case is discussed and the proposed mechanism of injury is the traction produced by a dilated intestinal loop with adhesions to the spleen by fibrin membranes. Moreover, a review about some issues in diagnosis, conservative management based on image studies and surgical management of splenic injuries is made.
