ABSTRACT

Trimalleolar fractures account for 7%-11% of ankle fractures and are generally caused by high-energy trauma. They are unstable fractures that require surgical repair in most cases as it is associated with a better functional outcome, so it should be considered in all patients, even those with comorbidities. In this type of fracture, the posterior malleolus compromise has the highest complexity, and it is associated with a worse prognosis. The management is still controversial, especially regarding the order in which malleolus should be repaired and the fixation materials to be used.
