Endothelial corneal rejection in a patient with penetrating keratoplasty, two non-simultaneous episodes following messenger ARN immunization against SARS-CoV-2 in San Luis Potosí

ABSTRACT
The SARS-CoV-2 pandemic decreased the corneal transplants performed annually. Since 2020, health organizations have developed campaigns against the virus, promote vaccination and the guidelines have been adjusting for the reactivation of transplant programs. Several ophthalmologic manifestations have been associated to the different types of vaccines against SARS-CoV-2. We report a case of a 28-year-old male patient with bilateral penetrating keratoplasty; the patient had two doses of BNT162b2 mRNA SARS-CoV-2 vaccine, 3 months later of the left eye surgery. After two days of each vaccine, he presented the following signs and symptoms: decrease in visual acuity of 4 lines in Snellen test and in anterior segment, Khodadoust line was found in the cornea, this feature is characteristic of acute endothelial rejection of the allograft in the left eye. The treatment with corticosteroids was successful in both episodes. This is the first case of two episodes of acute endothelial rejection in a patient with penetrating keratoplasty in the same eye, associated with immunization with BNT162b2 mRNA SARS-CoV-2. Doctors and patients with corneal transplant should know the secondary effects of the vaccination of SARS-CoV-2 to receive a treatment in a timely manner and guarantee the corneal graft survival and preserve life quality of the patient.
