Simulation for combat health team training: a different experience with an interdisciplinary and global approach

ABSTRACT

This manuscript summarizes an unpublished experience of a program implemented by the Chilean Navy, with the participation of other branches of the armed forces and civil organizations, based on a successful course in the United States, called Combat Casualty Care Course, better known by its initials "C4". Working as a team and with results of excellence in healthcare is a challenge. The healthcare professional is permanently exposed to making mistakes. Training these professionals requires different educational strategies and instances of repetition, to reach a minimum of quality and be able to function in very diverse contexts. Therefore, the education and training of specialties such as that carried out by the health team, in very complex conditions such as war conflicts and disaster situations, requires a very specific curriculum design, which guarantees the achievement of learning results. In combat, the healthcare personnel themselves are also exposed and may, on occasions, must decide between the mission or the patient, prioritize, combat or limit care. The C4, which has been held in Chile for 20 years, has brought together course instructors from the US C4 in Camp Bullis, San Antonio, TX, Chilean military, civilian, and Spanish faculty, and students from most Latin American countries, reaching great international prestige. Preserving the meaning of the original course, the C4 in Chile, has been characterized by integrating immersive simulation and a high level of standardization, realism and permanent evaluation since its inception, allowing each version to be better than the previous one and to be kept up to date with the best advances and experiences in trauma management in combat and disaster situations.
