ABSTRACT

Background: Monkeypox is an emerging disease that can range from a mild condition to serious complications, which have not been fully described.
Clinical case: A 32-year-old male patient, with HIV, who presented with monkeypox and abundant sudden-onset rectal bleeding and hypovolemic shock, six days after the onset of skin lesions. Colonoscopy showed a bleeding ulcer in the internal anal margin, despite this, the histopathological findings were inflammatory infiltrates and granulation tissue formation.
Conclusions: The monkeypox virus seems to have a variable behavior, being more serious in people with immunocompromise; however, some symptoms can alert us to possible complications for timely intervention.
