Rickettsiosis associated with multiple organic failure in the context of a pregnant woman. Case report and literature review

ABSTRACT

Rickettsia is a bacterium of the Rickettsiaceae family. It is characterized by causing fever, rash, spots on the skin and rarely has a serious evolution. Its vector is ticks and in southern Mexico its incidence is relatively low compared to the north of the country. Case presentation: a woman with a pregnancy of 37 weeks of gestation with late fetal death that evolved into septic shock with multiple organ failure.In Mexico, the incidence of this disease in the population obstetric is low; The highest number of cases have been reported in northern country. The diagnosis of this type of infectious diseases in the obstetric population is rare because it shares clinical symptoms with pathologies associated with hypertensive disorders; Mortality reported in those who are diagnosed is high at 40 to 80%, due to late diagnosis. In this clinical case, the symptoms were compatible with a hypertensive disorder of pregnancy; however, its evolution did not correspond to the course of this type of condition. Hepatic and sodium alterations and the patient's risk factors established the suspicion for Rickettsia; With laboratory confirmation, antimicrobial management was initiated, achieving improvement restoration of organ function and discharge from the service.
