Posterior downfracture of the maxillary in Le Fort I osteotomy

ABSTRACT
The main objective of this study is to present the technique of posterior descent of the maxilla, providing the maxillofacial surgeon with a versatile and dynamic technique with better results and fewer complications during the operation. Likewise, as secondary objectives, we present a series of cases where bleeding decreased significantly during surgery and the surgical time was shorter compared to the conventional technique, advantages that this surgical technique presented during the analysis of the cases.The Le Fort I osteotomy is one of the more complex procedures practiced in the field of maxillofacial surgery. Publications currently report intraoperative complications, which include bleeding, when using this surgical technique even though the procedure is widely used by maxillofacial surgeons. The nasal mucosa bleeding has not been considered, in spite of its importance on visibility during the procedure. Almost 95% of orthognathic publications mention conventional Le Fort I osteotomy. This article presents a modification of the surgical technique with the intent of improving the vision of the surgery field and the dynamics of the downfracture approach to ensure greater safety. Several cases of patients who have undergone surgery using the modified procedure are compared to cases of control patients to show that the proposed technique allows a noticeable decrease of total bleeding during surgery and a shortened length of surgery time.
