Hip-spine syndrome. A challenge in its diagnosis and treatment

ABSTRACT

Introduction: the hip-spine syndrome is the clinical scenario of symptoms where both hip osteoarthritis (OA) and lumbar pathology coexist, first described by Offierski and McNabb (1983). This radiating pain that affect the lower extremity is expected to become increasingly common due to the aging population and the consequent rise in the incidence and prevalence of OA. These overlapping symptoms can make its approach difficult, leading to failures in it, and consequently to unnecessary expenses, risks, and complications. Objective: propose an approachment algorithm for patients with symptoms in the hip-spine region, based on what has been published in recent years to avoid missing data as much as possible, so its application becomes easier, adaptable to clinical practice, aiming to achieve the best treatment outcomes with the lowest risk of adverse events for the patient, also optimizing the resources of institutions and and the surgeon's time. Material and methods: a systematic search was conducted in three databases on the diagnostic and therapeutic approach to hip-spine syndrome. Articles were analyzed based on previously established inclusion and exclusion criteria. Results: 16 articles were identified, and 18 additional articles extracted from other sources were included, covering a total of 34 articles, from which a literature review was written. Conclusion: this syndrome presents a challenge for its diagnosis, treatment, and prognosis, making it important to build up a systematic approach to its management and lay a basis to continue conducting research about the topic.
