L5-S1 fracture dislocation: case report and literature review

ABSTRACT

Lumbosacral dislocation is a rare and underreported pathology, due to its diagnostic and therapeutic complexity, it represents a real orthopedic challenge. We present the case of an 18-year-old female patient who, after a car accident, presented L5-S1 dislocation with associated fracture of the lumbar transverse processes upon admission to the emergency room with a neurological injury established in ASIA B, which was managed with open reduction, L4-S1 posterior instrumentation, L5 and S1 decompression and posterolateral fusion, presenting immediate improvement and recovery of muscle strength, with adequate postsurgical evolution. Follow-up is carried out 4 weeks after surgery, presenting in the outpatient clinic with residual sensory injury in the S1 dermatome, with subsequent referral to rehabilitation in our unit for reincorporation to walking and gain muscle strength.
