Renal fungal bezoar in an infant: a case report

ABSTRACT

Introduction: bezoar or fungal ball is a rare complications of fungal urinary tract infections, that occurs because of fungus colonization in a cavity, which generates hyphae accumulation. Objective: to present the medical and surgical treatment of a renal fungal ball in an infant with prolonged fever. Description of the clinical case: 11-month-old male infant with Dandy-Walker malformation with prolonged fever. Initially a maxilloethmoidal sinusitis and a Pseudomonas aeruginosa urinary tract infection were identified and 15 days treatment with broad-spectrum antibiotic was given. Due to persistent fever, an ultrasonography and urinary tract tomography were carried-out. The imagens revealed pyonephrosis and nephrolithiasis with urinary tract dilation. In the ureterorenoscopy, a purulent lesion compatible with a fungal bezoar was identified, and the diagnosis was confirmed by pathology. He was treated with drainage of the secretion and systemic and oral antifungal treatment (amphotericin, fluconazole) with resolution of the problem. Conclusions: fungal bezoars are a rare cause of pyelonephritis and renal abscesses in children, which can cause obstructive uropathy and kidney injury; imaging and medical and surgical treatment are necessary for complete resolution.
