ABSTRACT

Nicolau syndrome (NS), also known as embolia cutis medicamentosa, is a rare complication of intramuscular injections characterized by skin necrosis with a livedoid pattern. It is thought to result from perivascular damage caused by trauma during the injection. We present the case of a 76-year-old male who developed an ulcer with fibrin, granulation tissue, and hyperpigmented perilesional skin on the lateral gluteus following a vitamin B12 injection. After receiving appropriate treatment, the lesions fully resolved.
