Correlation of intracranial pressure through transcranial Doppler and intraparenchymal catheter in acute neurological injury

ABSTRACT

Neurocritical patients require close monitoring of intracranial pressure to ensure adequate cerebral perfussion pressure and thus be able to prevent the presentation of secondary lesions, reduce the impact on the morbidity and mortality of patients, so through the evaluation at the bedside of the neurocritical patient in the intensive care unit and it is through transcranial Doppler a non-invasive method with which the index is identified pulsatility of the middle cerebral artery in the M1 portion using a Vinno5 ultrasound with a 1.35-4.3 mHz G1-4P sectoral transducer and at the same time intracranial pressure measurement is performed by means of an intraparenchymal silicon catheter type Codman ICP Express placed by means of a craniotomy and a subdural screw, in order to correlate between the invasive and non-invasive method. Performing 948 measurements with a positive correlation of 100% between the normal pulsaltility index and intracranial pressure per intraparenchymal catheter and worldwide a single reference similar to our study by Dr. Johan Bellner in 2004 has been found, in which 81 patients with different neurological lesions were included that produced elevation of intracranial pressure evaluated with intraventricular pressure catheter versus pulsatility index, finding a correlation of p = 0.0001 similar to our study, however we wereable to establish a cut-off point to predictin tracranial hypertension with a pulsatility index of 1.32.
