Sphingomonas paucimobilis as a cause of community-acquired urinary tract infection in pediatrics?

ABSTRACT

Sphingomonas paucimobilis infections are rare, even more in children. It can cause infection in any organ. The infection can be acquired in nature or in the hospital setting. The course of the infection is mostly benign, with mortality being rare. In this paper we describe the fourth case worldwide, and the first in the American continent of urinary tract infection in pediatric patients due to S. paucimobilis. The case described is a six-year-old male who arrived at the pediatric emergency room with nausea, vomiting, and abdominal pain. On physical examination he was found to has low blood pressure. Empirical treatment with ceftriaxone was started after taking cultures, with good evolution. The report of the urine culture taken with a bladder catheter reported growth greater than 50,000 CFU of multisensitive S. paucimobilis. He was discharged on cefixime, and at the time of this writing he has been healthy on follow-up.
