Mamography coverage in Mexican women: estimation and analysis of associated factors

ABSTRACT
Objective. To estimate mammography coverage in Mexico and analyze the sociodemographic and spatial factors associated with mammographic screening. Materials and methods. A cross-sectional study was conducted. Mammography coverage among Mexican women aged 40 to 69 was analyzed using data from the National Health and Nutrition Survey (Ensanut) for the years 2018, 2021, and 2022. Associations between sociodemographic factors (age, marital status, education, indigenous language, area of residence, level of marginalization, and the presence of medical specialty units in the area) and spatial factors (Euclidean distance to medical services) were assessed using multilevel analysis. Results. The analytical sample included 11 227 women in 2018, 4 177 in 2021, and 3 659 in 2022. Mammography coverage in the previous year declined from 27.46% in 2018 to 16.03% in 2021, followed by an increase to 20.14% in 2022. Coverage was consistently higher in urban areas compared to rural areas, and in regions with lower levels of marginalization. Age, education, marital status, speaking an indigenous language, and distance to medical services were significantly associated with the odds of undergoing mammography. Conclusions. The findings reveals inequaities in access to mammography, influenced by individual and contextual sociodemographic factors. It is essential to strengthen and target strategies to improve coverage.REFERENCES
