Rosaceiform tuberculid of Lewandowsky: a challenging diagnosis

ABSTRACT
Cutaneous tuberculosis is a chronic infectious disease caused by
Mycobacterium tuberculosis. Nodular tuberculid of the face,
also known as Lewandowsky’s rosacea-like tuberculid, is a rare
variant of cutaneous tuberculosis, accounting for less than 1% of
all cases. We present a 47-year-old immunocompetent male with
a dermatosis characterized by papular and some nodular lesions in
the central facial region. Initially misdiagnosed as rosacea due to
inadequate response to standard treatment, a subsequent histopathological
study revealed tuberculoid granuloma formation. This
prompted a purified protein derivative (PPD) test, which was positive
at 72 hours, confirming the diagnosis of Lewandowsky’s rosacea-
like tuberculid. A search for systemic tuberculosis yielded no
evidence of extracutaneous involvement. The patient responded
favorably to anti-tuberculosis treatment.
