Monitoring the maximum epidermal temperature when using radiofrequency with the DORIAN® device: safety recommendations

ABSTRACT
Radiofrequency is used to tighten skin loose. During its use, the temperature of the epidermis and dermis must be monitored. Radiofrequency should be interrupted when the maximum cutoff temperature is reached, to avoid burns. The DORIAN
® device lacks thermometers and an automatic radiofrequency cutoff system. Therefore, the objectives of this study were: to monitor the maximum epidermal temperature during radiofrequency application with the DORIAN
® device and to define the maximum cutoff temperature and its safety range in relation to the maximum epidermal temperature. 30 participants were included; 4 maximum cutoff temperature levels were assessed in the infraumbilical skin to stop radiofrequency: in groups G1 (36.0 °C), G2 (37.5 °C), G3 (39.0 °C) and G4 (41.0 °C). Radiofrequency was applied of 48 W power using the DORIAN
® device. The maximum epidermal temperature was recorded using an infrared digital thermometer. A total of 120 measurements were analyzed, 30 in each study group. Monitoring showed an increase in epidermal temperature after radiofrequency was suspended at the maximum cutoff point defined for each group was reached as described below: G1 (36.0 °C), the maximum epidermal temperature reached 37.5 °C; in, G2 it ranged from 38 to 42 °C; 17 measurements in G3 (39.0 °C) reached 42 °C and 13 reached ≥ 43 °C, while all measurements in G4 (41.0 °C), had a epidermal temperature was ≥ 43 °C (p < 0.001). We conclude that this is the first study to define a cutoff temperature of 37.5 °C as a safety parameter when using the DORIAN
® device. It is recommended to discontinue radiofrequency application at 37.5 °C, as the heating continues to rise into the safe range of 38 to 42 °C. Surgeons must monitor epidermal temperature when using the DORIAN
® device.
