Graft survival after penetrating keratoplasty: three years of experience

ABSTRACT

Introduction: penetrating keratoplasty is a surgical procedure that replaces the total thickness of the patient's cornea with a donor graft, with the aim of better quality in the patient's vision. Objectives: to evaluate short-term survival outcomes and determine the prognostic factors of corneal transplantation performed at a tertiary referral hospital in Mexico. Material and methods: a longitudinal, retrospective, and analytical study of 51 patients who intervened in penetrating keratoplasty at a tertiary referral center of the Mexican Institute of Social Security was carried out between October 2017 and October 2020. This study included patients with ≥ 1 follow-up year. Results: 76.47% of the patients had an improvement in visual acuity. Survival at one year of follow-up of 74.50% of the grafts was found, with a mean survival of 10,824 months. Granular corneal dystrophy (Hazard Ratio (HR) 47.4, p = 0.004), leukoma (HR 7.1, p = 0.003), and vascularization (HR 7.4, p = 0.001) were identified as risk factors associated with failure of the corneal graft. Conclusions: in this study, granular corneal dystrophy, leukoma, and vascularity were independent risk factors associated with corneal graft failure in the Mexican population. New prospective studies are needed to follow up long-term and assess the clinical progression of patients who have received a corneal graft.
