Wünderlich syndrome, diagnostic approach and treatment in the emergency department

ABSTRACT

Wünderlich syndrome, although rare, represents a diagnostic and therapeutic challenge due to the multiplicity of possible etiologies and the nonspecific clinical presentation. Diagnostic complications are exacerbated by the patient’s hemodynamic instability, which also complicates the choice of the appropriate therapeutic approach. It is crucial to consider various factors in addition to hemodynamic stability, such as the integrity of both kidneys, the etiology, and the available resources. This article reports the case of a patient who presented to the emergency department with rapid hemodynamic deterioration secondary to a non-traumatic renal hemorrhage, the etiological cause of which was an angiomyolipoma. The diagnostic and therapeutic approach is discussed, highlighting the challenges and key decisions managing this condition.
