Laparoscopic bilateral pre-transplant nephrectomy in a patient with grade IV vesicoureteral reflux and chronic kidney disease

ABSTRACT

A bilateral transperitoneal laparoscopic nephroureterectomy was performed on pre-transplant natives through five ports with a change in modality from peritoneal dialysis to renal replacement, removing the Tenckhoff catheter through a mid-infraumbilical wound for the extraction of both native kidneys and placement of a Mahurkar catheter. Surgical time: 180 min, intraoperative blood loss 100 mL, hospital stay: 48 h. After 30 days, living donor kidney transplant performed, warm ischemia 4 min, cold ischemia time (CIT) 91 min, uremia. Discharged on the fifth postoperative day. Serum creatinine (Cr) 1 mg/dL, hemoglobin 6.7 mg/dL without transfusion requirement, maintained transurethral catheter at home for four days until urinary volumes decreased and intermittent bladder catheterization program was restarted. Follow-up four months post-transplant: Cr 1.14 mg/dL, hemoglobin 14.1 mg/dL.
