Clinical diagnosis of acute heart failure

ABSTRACT

Deep knowledge of the relevance of signs and symptoms of acute heart failure (AHF) would lead to better diagnostic and decision-making strategies. The signs and symptoms of congestion are easily assessed bedside. It is interesting to analyze the sensitivity, specificity and the importance of each of them in the diagnosis of AHF. More specific signs (jugular venous pressure) are more difficult to determine, and the agreement among physicians examining the same patient tends to be poor. Both the symptoms and the classic signs of AHF can have a high sensitivity (dyspnea) or specificity (in the case of orthopnea, paroxysmal nocturnal dyspnea), but not both at the same time. Given the limitations of these findings, it is necessary to find sup-port on other studies such as imaging tests and biomarkers for diagnostic confirmation.
