Quantification of donor septal cartilage by 3D segmentation in Mexican patients: useful area and volume preserving the L-strut

ABSTRACT

Septal cartilage is a key source for grafts in structured rhinoplasty. However, there are scarce morphometric data on the Mexican population. The objective of this study was to quantify the area and volume of the nasal septum in Mexican patients and to describe a computed tomography segmentation technique to estimate available cartilage. Twenty computed tomographies of adult patients (10 women and 10 men) were analyzed. Manual 3D segmentation was performed with the 3D Slicer software, respecting a 1 cm safety L-strut. Total septal area and volume, usable area, usable volume, and L-strut dimensions were measured. The average total septal area was 4.6 cm2 and the total volume 2.88 cm3. The average usable area was 3.2 cm2 and the usable volume 1.69 cm3. The L-strut showed an average area of 2.04 cm2 and volume of 1.16 cm3. Women showed significantly higher values in all measurements. Compared with Asian and European studies, the Mexican population presented larger usable septal areas but lower total volume than the European population. We concluded that Mexican patients have greater available septal cartilage area for grafts than other studied populations. This finding may represent an advantage in reconstructive procedures. CT segmentation is a useful tool for precise and personalized surgical planning.
