ABSTRACT

The pedicled thoracodorsal artery perforator flap (TDAP) is an island skin flap based on a cutaneous perforator of the thoracodorsal artery, which completely preserves the latissimus dorsi muscle and reduces the incidence of complications at the donor site. This flap has been used for reconstruction of the neck, elbow, arm, shoulder, axilla, pectus excavatum, breast, and other reconstructions. This study documents the case of three patients who underwent reconstruction with pedicled TDAP. For the design and performance of the flap, the technique described by Angrigiani was used. Three cases of reconstruction with this flap are presented: two women and one man; the reconstructed areas were shoulder, axilla, and arm; the patients' age range was 27 to 60 years. No postoperative seromas or flap necrosis were observed. Only one patient presented a small dehiscence of the donor area. The pedicled TDAP represents a safe alternative to the latissimus dorsi flap due to the low morbidity and low complication rate in the donor area. It is a valid reconstructive option for defects in the shoulder, arm, and axilla.
