Mature dermoid cyst with atypical presentation in pediatric age: case report

ABSTRACT
Introduction: germ cell tumors constitute a heterogeneous group of neoplasms comprising about 20 to 25% of all ovarian tumors. They mainly affect girls and young women, accounting for the majority of tumors in the pediatric population. The incidence of mature cystic teratomas is approximately 1.2-14.2 cases per 100,000 person-years. This tumors are the most common between the ages of 10 and 30 years and they may be incidental findings in some cases. Case report: we present the case of a 17-year-old female patient, high school student, native and resident of the state of Quintana Roo, nulligesta, nulliparous, without sexual debut, without chronic degenerative history referred, only with a history of conservative surgery of the right ovary for adnexal tumor in 2017 for mature ovarian teratoma without complications. Patient is referred to the corresponding unit and on admission with unique clinical findings: soft depressible abdomen, moderately painful, prominent lower abdomen on palpation with presence of mass of approx. 21 × 15 × 10 cm apparent origin from left adnexa. Exploratory laparotomy is performed subject to transoperative findings, which is carried out without complications. The histological results confirm the diagnosis idea: a mature cystic teratoma, a tumor measuring 19 cm in its largest diameter with integral capsule, edematous and congested salpinge. Conclusion: the clinical case presented here highlights the heterogeneity in the course of the clinical presentation that can occur occasionally in this type of neoplasm, as it was in our patient who had a history of a previous contralateral teratoma of smaller size with benign histology and debuts again with another adnexal mass of exaggerated proportions that takes out of context the classic characteristics of mature dermoid cyst; therefore, since the transoperative study was un available, which is the gold standard for diagnosing's benign or malignant tumors, conservative surgery of the affected gonad was not considered due, to the uncertain risk of it being a borderline lineage or with a high risk of malignant transformation according to the current literature.REFERENCES
