Diagnosis and surgical treatment of upper extremity compartment syndrome

ABSTRACT
The first description of upper extremity compartment syndrome is attributed to Richard von Volkmann in 1881. The normal pressure within the osteomyofascial compartment ranges from 0 to 10 mmHg. Compartment syndrome is defined as an increase in pressure in one or more osteomyofascial compartments exceeding 30 mmHg, surpassing perfusion pressure. The objective of this study is to share the experience at the Regional General Hospital No. 1 in Querétaro related to the diagnosis and surgical treating upper limb compartment syndrome. We performed a retrospective study from May 1, 2022 to May 1, 2025 on patients with this syndrome. A total of six patients underwent fasciotomy, four men (66.66%) and two women (33.33%), the mean age was 38 years (25-70 years), with 50% right-handed and 50% left-handed. 100% of the affected hands were dominant and the most frequent cause was crush injury from work accidents. Although our hospital lacks equipment to measure the osteomyofascial compartments, a clear understanding of the pathophysiology, clinical compartmental syndrome and surgical anatomy of the upper limb were crucial for patient's recovery.
