Quality of life in patients with endogenous hypercortisolism before and after surgery

ABSTRACT

Introduction: Chronic exposure to excess cortisol has a negative impact on quality of life.
Objectives: To determine the quality of life in patients with endogenous hypercortisolism to identify whether there is a relationship between this and clinical and hormonal variables in patients before and after surgery.
Methods: A prospective study of 25 patients with endogenous hypercortisolism was conducted. They were evaluated before and six months after surgical treatment. The CushingQoL, Tuebingen CD-25 and WHOQOL BRIF questionnaires were applied. For statistical significance, the mean, standard deviation and McNemar test were determined. Median quantitative clinical and hormonal variables and quality of life questionnaire scores before and after surgery were compared using Wilcoxon testing.
Results: 64 % had Cushing's disease. The median age was 46 years. The median sum of all domains of Tuebingen CD – 25 before surgery was 49 and then 27 points; of CushingQol was 41.6 and 62.5 after, and WHOQOL BREF before was 72 and after it was 78. Waist circumference and cortisol were positively correlated with BMI (r = 0.834; p = 0.000) and (r = 0.614; p = 0.001), respectively.
Cortisol was negatively correlated with psychological health (r = -0.472, p = 0.017) and social relationships (r = -0.401, p = 0.042). Conclusions: Quality of life was perceived as affected before surgery and improved considerably after surgery. There is a relationship between quality of life and some clinical and hormonal parameters.
