Knowledge, attitudes and practices of dentistry students regarding the management of patients under antithrombotic therapy

ABSTRACT
Introduction: the dental management of patients receiving antithrombotic therapy remains a clinical challenge due to the potential bleeding risk and the frequent misconception that medication must be discontinued before dental extraction. Current evidence indicates that most procedures can be safely performed without interrupting therapy when appropriate local hemostatic measures are implemented.
Objective: to evaluate the knowledge, attitudes, and practices of dentists regarding the management of patients under antithrombotic therapy, particularly concerning drug discontinuation prior to tooth extraction.
Material and methods: a cross-sectional observational study was conducted through a structured questionnaire distributed among dentists from various healthcare institutions. The
survey assessed the identification of anticoagulant and
antiplatelet agents, evaluation of bleeding risk, decisions
regarding therapy suspension, and management of
postoperative complications. Descriptive statistics
were used for frequency and percentage analysis.
Results: although most respondents identified the main
antithrombotic drugs, uncertainty persisted regarding
therapy discontinuation. Sixty-two percent considered it
necessary to suspend the medication before extraction,
and only 28% were aware of clinical guidelines
discouraging this practice. Forty percent reported
feeling insecure when treating these patients, and over
70% had not received specific training on the subject.
Conclusions: sufficient knowledge was identified among
dentists but inconsistent regarding the management
of patients under antithrombotic therapy. Lack of
awareness of current recommendations may increase
thromboembolic and some trans or postoperative
hemorrhagic risks, underscoring the need to strengthen
professional education in pharmacology and clinical
management.
