Oral manifestations of idiopathic thrombocytopenic purpura: case report

ABSTRACT
Introduction: idiopathic thrombocytopenic purpura (ITP) is an autoimmune disorder characterized by isolated thrombocytopenia and increased risk of bleeding. Oral findings may represent early warning signs, particularly when minor trauma is present.
Objective: to present a severe ITP case initially suspected due to oral manifestations.
Case presentation: a 75-year-old male presented with a denture-related tongue injury and persistent bleeding. Intraoral examination revealed ecchymosis, petechiae, and gingival bleeding; extraoral petechiae were also noted. Laboratory tests showed a critical platelet count of 1 × 103/µL. Urgent hematologic evaluation confirmed acute ITP. Treatment received with intravenous immunoglobulin and corticosteroids led to clinical improvement.
Conclusions: oral findings such as ecchymosis, petechiae, and persistent bleeding may indicate severe ITP. Dentists play a key role in early recognition and timely referral, contribuiting to the prevention of serious hemorrhagic complications.
