Ceftazidime-avibactam as a treatment option for carbapenemresistant gram-negative bacilli infections. Experience from two hospitals in Mexico City

ABSTRACT
Background. Carbapenem-resistant gram-negative bacilli can cause life-threatening infections, and treatment options are limited. Ceftazidime-avibactam has been shown to be an effective treatment against these microorganisms.Objective. To report the clinical, biochemical, and microbiological outcomes of ceftazidime-avibactam against carbapenem-resistant gram-negative bacilli infections (specifically Enterobacterales and Pseudomonas aeruginosa) in patients at two hospitals in Mexico.
Methods. This was an observational, retrospective study at two hospitals in Mexico City. All patients with carbapenem-resistant gram-negative bacilli infections who received ceftazidime-avibactam as definitive antimicrobial therapy were included. Microorganism identification and susceptibility were determined using the Vitek2 system, using CLSI 2019 breakpoints. The main clinical outcomes were clinical and microbiological cure.
Results. Seventy-nine cases of carbapenem-resistant gram-negative bacilli infections were analyzed. Infectious syndromes included: 40% urinary tract infection, 18% healthcare-associated pneumonia, 17% central venous catheter-associated bacilli, 15% postsurgical infections, and 9% complicated skin and soft tissue infections. Clinical cure was achieved in all cases of bacilli, healthcare-associated pneumonia, and skin and soft tissue infections, and in 87.5% of urinary tract infections. Microbiological cure was recorded in all cases of bacilli, 92% of urinary tract infections, and 72.7% of healthcare-associated pneumonia.
Conclusions. These results support ceftazidime-avibactam treatment for infections caused by carbapenem-resistant gram-negative bacilli. However, in our country, studies involving a larger population are needed to generalize these results.
