Prenatal testicular torsion: a case series and literature review

ABSTRACT

Prenatal testicular torsion is a rare condition with challenging diagnosis and low salvage rates. We report six neonates managed at a perinatal center over 10 years. Median gestational age was 37.4 weeks (interquartile range [IQR]: 36.5-38.3) and median birth weight 2,955 g (IQR: 2,850-3,250). Prenatal suspicion occurred in two cases. None presented with acute scrotum; all showed scrotal swelling and discoloration. Doppler ultrasound was performed in two patients, confirming absent blood flow. All underwent surgery within the first 24 hours (median: 6 h). Extravaginal torsion occurred in 83.3%. Orchiectomy was required in four cases (66.6%), while two (33.3%) had testicular preservation, with one progressing to atrophy. No complications were observed. Presentation is typically insidious, making clinical examination essential. Early surgery does not ensure viability; however, contralateral orchiopexy remains crucial to prevent anorchia.
