ABSTRACT

Intermittent Left Bundle Branch Block (LBBB) is an uncommon clinical entity (prevalence <1%), characterized by dynamic alternation between normal conduction and complete left bundle branch block. This is illustrated in the case of an 81-year-old male whose recording shows intermittent episodes of LBBB alternating with sinus rhythm throughout. This report investigates this understudied condition and explores how its clinical and prognostic characteristics can facilitate the screening of other cardiac pathologies. Key pathophysiological mechanisms include phase 3 block, the linking phenomenon, and "cardiac memory"—which can be mistaken for myocardial ischemia. Given that its presence at low heart rates predicts a higher risk of cardiovascular events and mortality, we conclude that this disorder represents a significant opportunity for electrophysiological research. Its identification is essential for assessing the risk of remodeling or arrhythmias in patients with incipient or asymptomatic heart disease.
