ABSTRACT
Parapneumonic pleural effusion is a common complication of community-acquired pneumonia and may progress to empyema if left untreated. This case report presents an 18-year-old adolescent with complicated pleural effusion caused by Streptococcus β-hemolytic group G, an uncommon pathogen. Despite initial management with antibiotics and pleural drainage, the patient developed loculated collections, requiring surgical pulmonary decortication.
The objective of this report is to highlight the importance of continuous clinical assessment and a multidisciplinary approach in adolescents with complicated empyema. Current literature supports a stepwise treatment approach, although the optimal timing for surgical intervention remains debated. Pulmonary decortication has proven effective in restoring lung function in advanced cases, particularly when performed promptly.
This case underscores the need for close monitoring in young patients who do not respond to conservative therapy and reinforces the role of thoracic surgery as a safe and effective treatment strategy in refractory pleural infections.
