ABSTRACT

Endotracheal extubation is a critical moment during anesthesia, particularly in pediatric patients, due to their increased risk of respiratory complications. This literature review analyzes the available evidence on two extubation strategies: deep anesthesia extubation and awake extubation. Nine relevant studies published between 1990 and 2023 were included. Deep extubation has been associated with reduced incidence of coughing, agitation, and laryngospasm, while awake extubation provides better airway reflex control. No clear superiority was found between the techniques. The choice should be individualized based on patient characteristics, surgical procedure, and anesthesiologist's experience.
