Diffuse gastric adenocarcinoma in an adolescent

ABSTRACT

Introduction: gastric adenocarcinoma (GAC) is rare in the pediatric population, representing less than 0.1% of cases. The low incidence and nonspecific symptoms delay diagnosis. The aim of this report is to describe a case of an adolescent with diffuse GAC. Case description: a 17-year-old female patient presented with abdominal pain, vomiting, weight loss, and constipation. Imaging studies revealed liver lesions, diffuse gastric thickening, and bilateral ovarian masses. Endoscopy with biopsy confirmed diffuse, poorly cohesive giant cell gastric carcinoma with signet ring cells. Hepatic, peritoneal, bilateral ovarian (Krukenberg type), and brain metastases were confirmed. She required multiple surgical interventions and intensive care management; however, she rapidly progressed to multiple organ failure and died. Conclusions: GAC in children and adolescents presents a diagnostic and therapeutic challenge. This case highlights the importance of early suspicion as part of the differential diagnosis in patients with gastrointestinal symptoms.
