Coexistence of androgenetic alopecia and alopecia areata: a case report

ABSTRACT

Alopecia is defined as the absence or loss of hair in areas where it is normally present. It stands as one of the most common reasons for trichological consultation and is frequently associated with psychological distress including anxiety and depression, which significantly diminishes the patient’s quality of life. Both androgenetic alopecia (AGA) and alopecia areata (AA) are primary etiologies of non-scarring hair loss. While often regarded as distinct entities, current evidence suggests that their coexistence is not uncommon, potentially complicating both the diagnostic approach and therapeutic response. We present the case of a 44-year-old male with an 8-month history of hair loss in the occipital region. Clinical and trichoscopic features revealed the simultaneous presence of AGA and AA. A combined treatment regimen utilizing topical and oral minoxidil, alongside topical and oral steroids and methotrexate, successfully led to hair regrowth in the affected areas. In the discussion, we analyze the shared characteristics of these conditions, the pivotal role of diagnostic trichoscopy, and integrated treatment strategies.
