Meningitis secondary to Listeria monocytogenes, pathophysiology, clinical evaluation and case report

ABSTRACT
Listeria monocytogenes is a Gram-positive, intracellular, aerobic
and facultative anaerobic bacterium. Although infections are uncommon,
it has high rates of hospitalization and mortality. Due to its significant
morbidity and mortality in at-risk populations, it is considered
a public health concern. According to the MONALISA study, the most
common manifestations are meningitis and meningoencephalitis in
susceptible patients: pregnant women, the elderly, and immunocompromised
individuals.
We present the clinical case of a male patient who developed
meningitis secondary to Listeria monocytogenes infection. He initially
presented with nonspecific symptoms: headache, fever, and decreased
muscle strength in the lower extremities. A cranial CT scan
revealed subarachnoid hemorrhage in the frontal sulcus. After an
unfavorable clinical course and subsequent onset of focal neurological
deficits, he was admitted to a private hospital. Based on his
initial progression, a lumbar puncture and specific tests, including
FilmArray, were performed, confirming infection with Listeria monocytogenes.
This case report analyzes the pathophysiology, diagnosis, and
treatment of listeriosis.
