Necrotizing pneumonia caused by Fusobacterium nucleatum after kidney transplant: a case report

ABSTRACT

Introduction: anaerobic bacterial infections such as Fusobacterium nucleatum are an underestimated cause of morbidity and mortality in kidney transplant recipients. Case report: a 32-year-old male with a recent deceased-donor kidney transplant under immunosuppression (tacrolimus, mycophenolate, and prednisone) presented with dyspnea, lipothymia, and septic shock secondary to refractory left basal pneumonia. Initial cultures reported Klebsiella oxytoca ESBL (+). Due to clinical worsening despite broad-spectrum therapy, he was scheduled for lobectomy but died before the procedure. Post mortem MALDI-TOF MS identified Fusobacterium nucleatum as the causative agent of necrotizing pneumonia. Conclusions: necrotizing pneumonia caused by F. nucleatum is a rare and lethal opportunistic complication. Early diagnostic tools like MALDI-TOF MS are vital for directing targeted antimicrobial therapy in immunocompromised patients.
